Thank you for choosing The Extra Club Limited (www.theextraclub.com). Quick reminders, please:

1. Follow the latest Covid regulations required by the local government. If you have symptoms, even you are tested negative, please do not attend our event to protect yourself and the others.

2. Sign waiver below before your first wellness event with The Extra Club.

Wellness Event Procedures

Wellness event may occur in person, by phone, through video conference, or over email, depending on the venue that works best for the Client and what coaching package is selected.

You, as our valued client/ participant, agree to adhere to established appointment times and location.
You, as our valued client/ participant, agree to begin and finish on time. If you are 15 minutes late to an appointment, we, The Extra Club Limited, will assume that the appointment is cancelled and you will be responsible for the full fee.
If, we, The Extra Club Limited, is more than 15 minutes late to an appointment, you may assume that the event is cancelled and the shall not be responsible for any payment for that event.
Any changes or cancellations within 24 hours are subject to a 100% cancellation fee.

Photography at wellness event

The wellness coach/ facilitator/ producer of The Extra Club may ask participants to put away their phones/ cameras at certain periods to ensure the all participants are safe and have an enjoyable experience.
The Extra Club photographers (if arranged) will ask for consent of the participants if they would like to be photographed before the wellness session commence. The Extra Club appointed photographer(s) are professionals who will take photos balancing safety, privacy, participants experience and aesthetics of the photos.
Photos taken by the photographer(s) of the wellness event are owned by the photographer(s). The photographer, the wellness coach(s) and The Extra Club reserved the rights to use the photos to promote other services.
Photos taken by the photographer(s) of the wellness event may be distributed to the participants of the event. The participants have the usage right of the photos and shall cite the photos are taken at the wellness event produced by The Extra Club.

Fees

Fees as agreed by the client and The Extra Club are payable to The Extra Club prior to the wellness event. The payment can be settled via the following methods.

Waiver - Please sign this waiver if it is your first time participating an event organised by The Extra Club Limited. If you do not wished to be photographed or recorded, please let us know at the event. Thank you.

1. I am above the age of 18 and I am not aware of any injury, illness or other health related issues that would restrict or limit me to participate the event.

2. I agree to assume all risks and expenses due to an injury that may occur as a result of my involvement in the events organized by The Extra Club Limited.

3. I agree to hold The Extra Club Limited or anyone acting on its behalf either as a coach a coaching assistant or administrator harmless in the event of an injury to me while participating under the supervision of the event.

4. I understand the event is subject to weather condition and might be cancelled last minute/ rescheduled.

5. I consent and agree that The Extra Club Limited has the right to take or use photographs of me (and/or my property) and to use these in any and all media worldwide including online, now or hereafter known, and for any purpose whatsoever. I agree that any uses described may be made without compensation or additional consideration of me.
